First Time Patient Forms

I look forward to meeting you and working with you. Please call me before filling out these forms, so we can talk to make sure we are a good fit for a first session. Please fill out our Agreement and Intake forms. These are standard forms and necessary according to the American Psychological Association. The release of Confidentiality form is only necessary if you want me to talk with someone else, such as another doctor.

Please also email a copy of both sides of you insurance card along with your date of birth for verification to Jennifer (Dr. Maurer’s biller) at precisionbills@gmail.com.
